- W2464783721 abstract "Introduction: Inhaled steroids is considered the most effective and safe medication for control of persistent asthma in adults and children. Patients with severe asthma may present corticosteroid resistance, with different clinical presentations. Mechanisms of steroid resistance have been studied in adult asthmatics over the last decades. However, glucocorticoid (GC) resistance has not been studied in children with asthma. The aim of the present study is to analyze peripheral blood mononuclear cell (PBMC) sensitivity to GCs of children with severe persistent asthma. Methods: Children with persistent asthma (between 6-15 years of age) were divided into three groups (severe, moderate and mild), and compared with healthy children. Lung function tests and skin prick tests were performed in all studied asthmatic children. PBMCs were isolated and cultured in vitro to assess mitogen-induced proliferation as well as cellular sensitivity to dexamethasone. Results: Fifty-seven children with persistent asthma and 18 healthy children were included. Patients with mild asthma showed significantly less proliferation of stimulated T-cells (p<0.05). Moreover, only subjects with moderate asthma presented with lower lymphocyte proliferation when compared to the control group (p<0.05). When dexametasone sensitivity was analyzed, there was no difference between patients with asthma and controls. However, subjects with mild asthma showed less sensitivity to dexametasone (p<0.05) Children with severe asthma presented similar dexametasone sensitivity when compared to controls Conclusions: Children with severe asthma are not resistant to GCs from in vitro PBMC analysis. Our findings suggest that the resistance to GCs in adults with severe asthma might be an acquired process with aging process.
