- W2465162416 abstract "Individuals with lower extremity amputations, particularly transfemoral amputations (TFA), are at a ten times greater risk for knee osteoarthritis (KOA) in their sound limb than non-amputees. Mechanical loading is thought to play an important role in the etiology of KOA development and progression in the sound limb and can be assessed biomechanically. High sound limb external knee adduction moments (EKAM) and loading rates (LR) are common in older individuals with amputation, but recent work on individuals with transtibial amputations showed that young, active individuals demonstrate few biomechanical risk factors for KOA across a range of walking speeds. However, it remains unknown if biomechanical risk factors for disease development are present in young individuals with TFA. PURPOSE: To compare mechanical loading across a range of walking speeds in individuals with and without a unilateral TFA. METHODS: 14 male service members (mean age 29.7 years) with unilateral TFA and 14 controls (mean age 27.9 years) matched on leg length underwent gait analysis at a self-selected speed and at 3 standardized speeds based on leg length (mean speeds: 0.95, 1.21 and 1.45 m/s). 26 cameras recorded motion capture data (120 Hz) and force platforms recorded analog data (1200 Hz). The peak EKAM and flexor moment (EKFM), the EKAM impulse, and the maximum LR of the vertical ground reaction force were calculated. All dependent measures were normalized by body mass. A two-way ANOVA (group x speed) with Bonferroni-Holm post-hoc tests assessed statistical significance of all dependent measures. RESULTS: Self-selected speeds were not different between groups (TFA: 1.28±0.10 m/s, control: 1.31±0.09 m/s, P=0.349). The peak EKAM and its impulse were on average 25.7% and 27.1% lower, respectively, in the TFA group than controls across speeds (all P<0.014). LR, however, was on average 51.6% greater in the TFA group (P<0.001). CONCLUSIONS: At equivalent speeds, individuals with TFA in the early stages of ambulation in a prosthesis do not exhibit biomechanical risk factors for high medial compartment or axial loads (peak EKAM, EKAM impulse and EKFM), but do have high LR. A high LR may predispose individuals with TFA to cartilage or other tissue damage, although it may not be localized to the medial compartment of the tibiofemoral plateau." @default.
