- W2466475753 abstract "Beta-amyloid (Aβ) peptides, particularly Aβ40 and Aβ42, are directly involved in the pathophysiology of Alzheimer's disease (AD) and the alteration of their levels in the brain, measured either by amyloid PET (A-PET) scans or CSF analysis, is considered one of the earliest observable changes related to AD. However, both PET and lumbar puncture for CSF studies present problems that limit their widespread acceptability in global clinical trials. Thus, development of other measures as potential surrogates of brain amyloid positivity should be pursued, particularly for the pre-symptomatic stages of AD. In this work we show that the ratio between total Aβ42 and total Aβ40 concentrations in plasma (TP42/40) from cognitively healthy individuals is associated to larger values A-PET measurements, and also to a faster progression of brain fibrillar Aβ deposition. This longitudinal study included 162 healthy controls (HC) at baseline from AIBL dataset. We received plasma samples corresponding to 3 visits, at months 18, 36 and 54. A-PET scans were performed every 18 months on 128 subjects out of these HC individuals at several visits (median 4, min=1, max=5 visits). PET measurements were coded using a Becket score to make PIB and flutemetamol scans comparable. Total plasma levels of Aβ40 and Aβ42 were measured with ABtest ELISAs (Araclon Biotech. Zaragoza, Spain). For the cross-sectional (visit by visit) study, Kruskal-Wallis analysis was used to assess associations between plasma TP42/40 and A-PET measurements. Linear mixed-effect models were used for the longitudinal study. Cross-sectional: TP42/40 was significantly lower for PET-positive (Becket score cutoff=1.496) subjects than for PET-negative subjects at each visit (p=7.1e-4 at 18M, p=2.0e-3 at 36M, p=7.3e-6 at 54M). Longitudinal: Plasma TP42/40 measurements at M18 were significantly correlated with both baseline A-PET measurement (p=3.3e-4) and with the slope of A-PET Becket score time course (p=2.0e-4). Low values of TP42/40 in healthy individuals are associated with high amyloid PET and also with faster progression of brain amyloidosis during follow-up. Thus, plasma TP42/40 has potential for being considered as a cost-effective and non-invasive biomarker for AD as well as a useful tool for recruitment in early stage clinical trials." @default.
