- W2469793414 abstract "The use of blood flow restriction (BFR) with exercise has led to a variety of different protocols as well as different restrictive pressures used during these protocols. Many protocols apply the same restrictive pressure regardless of differences in limb circumference. This is problematic given that previous studies indicate that limb circumference is the largest predictor of arterial occlusion. However, these studies were completed in the supine position and most exercise with BFR is performed standing. PURPOSE: Thus, we sought to determine if limb circumference should still be accounted for when setting the BFR cuff pressure in the upper body when the participant is standing. METHODS: Participants had their circumference measured on the upper right arm at the midpoint between the acromion process and olecranon process. After fifteen minutes of seated rest, brachial systolic (bSBP) and diastolic (bDBP) blood pressure measurements were taken with an appropriate sized cuff. Standing arterial occlusion pressure was then measured to the nearest 1 mmHg using a narrow (5 cm) nylon restriction cuff applied to the most proximal portion of the right arm and a hand held bidirectional Doppler probe placed on the radial artery at the wrist. Two different models of hierarchical linear regression were used to determine the greatest predictor of arterial occlusion in the upper body. The first model was comprised of arm size and the second model included both arm size and bSBP. Standard error of the estimate (SEE), standardized betas (β) and part correlation coefficients (part) were used to investigate each model. Statistical significance was set at p ≤ 0.05. RESULTS: 31 participants (19 male, 12 female) between the ages of 18-32 years completed the study. The first model (SEE=9.7) significantly explained 61% of the variance in arterial occlusion with arm circumference. The second model (SEE=8.9) significantly explained 69% of the variance in arterial occlusion with arm circumference (β=0.639, part=0.568) explaining more than bSBP (β=0.312, part=0.277). CONCLUSIONS: Our findings indicate that arm circumference and bSBP should be taken into account when determining BFR cuff pressures. Additionally, the model suggests that restrictive pressures should largely be based on arm circumference (greater pressure for bigger arms)." @default.
