• W2474638934 abstract "The aim of the study was to identify clinical microbiological criteria for the necessity of removal of infected epithelium in the periodontal pocket in patients with periodontitis. A total of 90 patients diagnosed with periodontal disease of varying severity were assessed. Clinical examination of the patients was performed, including the definition of index numbers and microbiological diagnosis of the content of periodontal pocket and periodontal tissues by biopsy culture seeding. Epithelial integration of lactobacilli and bifidobacteria was established, indicating the need for a differentiated approach to the removal of gingival epithelium. Diagnostic criteria for deepitalization was identified: marked inflammatory response of periodontal tissues, characterized by performance indexes PMA > 50%, Mulleman > 1.5, Svrakov > 2.7, correlated with the degree of contamination biopsy 4IgKOE/ml and Candida spp. 2IgKOE/ml.Лечение пародонтита предусматривает удаление инфицированного эпителия тканей пародонта. Диагностические критерии удаления эпителия методом лазерной деэпителизации на этапе инициальной терапии не установлены. Цель исследования - установить клинико-микробиологические критерии необходимости удаления инфицированного эпителия пародонтального кармана (ПК) у пациентов с пародонтитом на этапе инициальной терапии пародонта. Материал и методы. Обследованы 90 человек с диагнозом пародонтита разной степени тяжести. Проводили клиническое обследование пациентов, включающее определение индексных показателей и микробиологическую диагностику содержимого ПК и десневого биоптата. Результаты. При прогрессировании пародонтита установлены увеличение индексов Мюллемана, РМА и йодного числа Свракова и эпителиальная интеграция лактобацилл и бифидобактерий, свидетельствующая о необходимости дифференцированного подхода к удалению эпителия с использованием лазера. При прогрессировании пародонтита увеличивалась концентрация совокупной микробиоты содержимого ПК с 5,1 до 6,2 lgКОЕ/мл, биоптата - с 3,7 до 6,2 lgКОЕ/мл и Candida spp. биоптата - с 0,8 до 3,6 lgКОЕ/мл. Определены диагностические критерии необходимости проведения лазерной деэпителизации на этапе инициальной терапии пародонта: концентрация совокупной микробиоты биоптата более 4 lgКОЕ/мл, Candida spp. биоптата более 2 lgКОЕ/мл, коррелирующие с индексами Мюллемана более 1,5 балла, РМА более 50% и йодного числа Свракова более 2,7 балла." @default.
