Three categories of respiratory system dysfunction can justify long termNPPV: an increase in respiratory load (due to intrinsic cardiopulmonary disorders orskeletal deformities); respiratory muscle weakness (due to neuromuscular diseases orspinal cord injury); or failure of neurologic control of ventilation (such as the centralhypoventilation syndrome). In these different diseases, the role of NPPV will be torespectively unload the respiratory muscles, to replace the respiratory muscles or toreplace central drive, in order to correct alveolar hypoventilation. The benefit ofNPPV on nocturnal and daytime gas exchange has been demonstrated in children.Other effects, such as an improvement of lung function, respiratory muscleperformance or respiratory mechanics, are less well documented. The effect ofNPPV on lung growth is an important point that needs to be investigated. The type ofequipment and the specific ventilator settings that should be chosen remain a matterof debate and evolve more rapidly with industry capability than with clear indicationsavailable from scientific trials. The major advantage of NPPV is that it can be appliedat home, combining greater potential for psychosocial development and familyfunction, at a lesser cost. The use of home NPPV requires appropriate diagnosticprocedures, appropriate titration of the ventilator, co-operative and educated familiesand a careful, well-organised follow up.
