In the US, a fragmented delivery system delivers highly variable non-measured services that are resistant to standardization, even if they are justified by strong evidence. This leads to suboptimal performance, with large gaps between evidence-based best practices, impacting patient health and well-being. Translation of evidence-based best practice into clinical practice is fraught with problems and barriers. The role of the care provider is important, but it has been shown that interventions directed toward both providers and the organizations in which they practice are more likely to succeed. While the information necessary to realize the promise of personalized medicine may be more complex and voluminous compared to other health information, with few exceptions the barriers to implementation are no different for genetic and genomic interventions. This chapter will explore the tolls of implementation science and the role of the health system in the delivery of personalized medicine using the example of implementation of a tumor-based Lynch syndrome screening program in an integrated healthcare delivery system." @default.
