- W2483583732 abstract "Advancing Parkinson’s disease (PD) is associated with a variety of motor andnon-motor symptoms that are quite unique to this illness and pose difficultclinical challenges. Most patients with advanced PD would benefit fromhaving a neurologist specializing in movement disorders as part of their careteam. The majority of patients eventually develop motor complicationsrelated to disease progression and dopaminergic pharmacotherapy (Table3.1). Motor complications include dyskinesias, wearing-off of medicationeffect prior to the next scheduled dose and unpredictable shifts in mobilityreferred to as the “on-off” phenomenon. Some patients also develop fluctua-tions in emotional states (e.g. mood, anxiety) and other non-motor domains(e.g. pain, autonomic function). In addition to these fluctuating symptomsgenerally thought to be secondary to dopaminergic dysfunction, patientsdevelop impairments that appear to be less related to dopamine neurotrans-mission, namely dysarthria, dysphagia, and impairments in gait and balance(Table 3.2).
