Concerns that the mentallydisordered may be remanded more frequently thantheir mentally well counterparts have led to thedevelopment of schemes for avoiding taking peopleinto custody. The Review of Health and SocialServicesfor Mentally Disordered Offenders and OthersRequiring Similar Services (HMSO, 1993) hasidentified, as a priority, research into the prevalenceof mental disorder among remand prisonersinEnglandand Wales. There are compelling reasons for anequivalentstudyinScotland,includingvariationsinpopulationdemographics,availabilityofpsychiatricservices and differences in the criminal justice andpenalsystems.We describethefindingsofasurveyofScottish remand prisoners in which we identified casesof major mental illnesson the basis of clinicalcriteria,as well as morbid levels of less severe symptoms,using a standardised semi-structured interview.MethodIn September 1993 nine institutions in Scotlandhoused prisoners awaiting trial: seven prisons foradult males, one women's prison and anotherexclusively for males aged between 16 and 21. Ateachestablishmenta50% randomsampleofalluntried prisoners was selected from lists of hallresidents. Printed information was distributed topotential subjects 48 h before the researcher's visit,and selected inmates were approached on the day ofthe study and the nature and purpose of the projectexplained to them. Emphasis was placed on voluntaryparticipation and confidentiality. Written consentwas obtained from participants. Prison medicalrecords of non-participants were examined whereavailable.Interviews were conducted by 12 psychiatrists(teams of four in larger institutions and two in thefive smaller jails). They each lasted 20—30mm, butwere extended where necessary. Practice sessions hadbeen used to establish consistent technique andscoring. Sociodemographic and historical data werecollected using a specifically designed schedule whichincluded information about the current charge,personal and family background, past medical,psychiatric and forensic history and use of alcohol(Mayfield et al, 1974) and drugs. Subjects wereexamined using the Clinical Interview Schedule(Goldberg et al, 1970), and two tests of cognitivefunction, the Quick Test (Ammons & Ammons,1962), and the Word Recognition Subtest of theBritish Abilities Scales (Elliot et a!, 1983). Clinicaldiagnoses were based on ICD—10criteria. Nomeasure of personality was included.ResultsThe average weekly population in Scottish prisonsawaiting trial in September 1993 was 938. Weapproached 455 individuals: 58 did not wish to takepart, and eight initially selected were discharged fromcustody prior to interview. The 389 participantsconsistedof371menand18women,meanage25.8years (range 16—72);95.5% were native Scots. Thelength of time on remand varied from less than 24 h545" @default.
