- W2485114889 abstract "Brazil, the sixth largest world economy, has a highly heterogeneous and admixed population in excess of 200 million, with biogeographical Amerindian, European, and Sub-Saharan African ancestral roots. These characteristics represent a caveat concerning the extrapolation of pharmacogenetics-genomics (PGx) data from comparatively more homogeneous populations. And it has prompted the creation of the Brazilian Pharmacogenetics Network–Refargen, a consortium of 20 research groups whose mission is to provide leadership in PGx research and education in Brazil and ultimately to improve the health of its population. A major challenge for Brazilian PGx studies is population stratification, which if not controlled for, may confound the PGx association study outcomes. Refargen researchers have developed ways to deal with this challenge by combining ancestry-informative markers and appropriate statistical approaches. They have conducted PGx clinical trials involving a range of therapeutic medicines, including antipsychotics, antiretrovirals, and antituberculosis agents, clopidogrel and statins, and immunosuppressants in renal transplant recipients; L-thyroxine in thyroid cancer patients; methylphenidate in attention-deficit hyperactivity disorder; nonsteroidal anti-inflammatory agents; and warfarin. We review the major outcomes of some of these studies, and we highlight the development of PGx-dosing warfarin algorithms by our two groups. Adoption of these algorithms in the natural clinical setting of the Brazilian public health system and private practice faces the same barriers encountered worldwide, not only for warfarin but for PGx-informed prescriptions in general.
