- W2485889882 abstract "Sometimes accidents are said to be due to equipment failure, but equipment failure is due to errors by designers or errors by those who operate, install, or maintain the equipment. Some accidents are said to be due to organizational error, but organizations, like equipment, have no minds of their own. The term organizational errors is a euphemism for errors by managers who fail to recognize the weaknesses in organization or to take action to change it. Often these weaknesses are never designed or intended but just arise by custom and practice and become the common law of the organization. Managers and designers and those who advise them can make errors of four types, namely, slips and lapses of attention, violations, mistakes, and mismatches. Slips and lapses of attention are unlikely, as these executives usually have time to check their decisions; most errors by designers and managers are mistakes—that is, they are due to inadequate training or instructions, but some are due to violations or mind-sets. This chapter also describes accidents caused by those slips and lapses of attention that even well-trained and well-motivated persons make from time to time. For example, they forget to close a valve or close the wrong valve. They know what they should do, want to do it, and are physically and mentally capable of doing it, but they forget to do it. Exhortation, punishment, or further training will have no effect. Organization must either accept an occasional error or change the work situation to remove the opportunities for error." @default.
