- W2489490176 abstract "Publisher Summary This chapter describes the components and pathways of renin–angiotensin–aldosterone system (RAAS). The RAAS is a major hormonal cascade in the control of blood pressure (BP), hypertension, and tissue damage. The primary means by which the RAAS contributes to acute changes in extracellular fluid volume and BP homeostasis is by adjusting the level of renin in the circulation. Angiotensin II, the major effector peptide of the renin–angiotensin system (RAS), binds to two major receptors, angiotensin receptor (AT1) and AT2, that generally oppose each other. During the past five years, several new pathways in the RAS have been discovered and/or clarified, including a (pro)renin receptor, the functional properties of the AT2 receptor, and the angiotensin-converting enzyme-2–angiotensin (1–7) mas receptor pathway. The circulating RAS is only one component of the overall RAS, and tissue RASs that operate independently of the circulating RAS have been identified and characterized, especially in the kidney. RAAS blockade has been central to our treatment of hypertension and its complications such as heart failure, atrial fibrillation, and coronary artery disease and the prevention of progression to renal failure. Renin inhibition constitutes a promising new therapeutic target in RAAS blockade." @default.
