If a patient has the defining criterion, the diagnosis of reflux is definite (no other disease is possible), and if the diagnostic criterion is absent, the patient has no reflux disease. Classical symptoms are a commonly used defining criterion. If heartburn and regurgitation are used at a frequency more than occasional, it is a very specific criterion of reflux disease. However, there is no standard symptom combination or symptom frequency to define reflux disease. Erosive esophagitis is even less sensitive for reflux disease than classical symptoms. Erosions are less specific as a single criterion than symptoms because they may result from infections, pills, and corrosives, suggesting that the use of erosive esophagitis is worse than classical symptoms and therefore useless as a criterion to define reflux. Erosive esophagitis is used to grade the severity of reflux disease. Of the several classifications that exist, the one most commonly used for grading erosive esophagitis is the Los Angeles classification, which has been shown to have an excellent correlation with esophageal acid exposure as assessed by a 24-hour pH test." @default.
