- W2492400970 abstract "Despite progress in diagnostics, prevention, and treatment modalities, cardiovascular disease (CVD) remains the leading cause of combined morbidity and mortality in Western industrialized countries. Significant advances in genomics, transcriptomics, proteomics, and metabolomics, which have gained deep insights into the genetic causes and the mechanisms of multifactorial CVD, have opened up new prospects for two innovative therapeutic strategies: gene- and cell-based therapies. These novel modalities promise to fill the gaps and overcome shortfalls in the present CVD therapy to further improve care of patients with CVD. Various vectors, exogenous and endogenous stem cells (SCs) of noncardiac and cardiac origin, and new delivery strategies have been examined in the experimental and clinical settings. Due to the remarkable progress in the understanding of the molecular mechanisms underlying CVD, different potential targets for cardiovascular gene therapy have been suggested. They include components implicated in β-AR signaling, Ca2+ cycling, the cardiac conducting system, and cell survival and death. The discovery that various populations of cardiac progenitor cells (CPCs) are present in the adult human heart and that it possesses limited regeneration capacity has opened a new era in cardiac repair. Importantly, unlike bone marrow (BM)-derived SCs, autologous CPCs from myocardial biopsies cultured and subsequently delivered by coronary injection to patients have given positive results. Several clinical trials of gene- and cell-based therapy for CVD are now in progress. Although the results are encouraging and these technologies are safe, many hurdles need to be overcome before gene- and cell-based therapy become clinically relevant procedures." @default.
