- W2502097184 abstract "Syphilis, caused by the bacteria Treponema pallidum , is an important sexually transmitted infection (STI) for women, and is readily transmitted between sexual partners. Transmission to the fetus is associated with high fetal morbidity, spontaneous abortion, and stillbirth. Although syphilis is easily treatable with inexpensive and widely available antibiotics (penicillin), particularly in its early stages, recent epidemics have occurred in the US, China, Russia, Europe, and Africa. These epidemics result largely as a consequence of risky behaviors and sexual networks among men who have sex with men (MSM) in the US and Europe, social disruption, exchange of sex for drugs or money in Eastern Europe, and lack of appropriate health services for susceptible and infected persons in some settings. Clinically, signs and symptoms of the disease are often absent, making detection difficult and allowing for sexual transmission to occur unimpeded. The presence of the genital ulcers caused by T. pallidum promotes acquisition of the human immunodeficiency virus (HIV), so that this treatable STI likely provides more fuel for the global AIDS pandemic. Efforts to control syphilis include serological screening of susceptible persons (especially pregnant women and individuals with other STIs) and are largely responsible for the marked decline in incidence in the US over the past two decades. However, while the disease has become highly localized in the US, global elimination will require substantial investment in addressing the basic socioeconomic inequities that perpetuate this infection. Moreover, a resurgence of syphilis among MSM in many industrialized countries likely poses an increased risk for women who are sexually exposed to those MSM who also have sex with women." @default.
