• W2506316609 abstract "This research-in-progress considers some of the conceptual and practical challenges relating to the development, deployment and evaluation of information systems that have emerged as part of the Pathways Home Project. This project aims to support community-based patients suffering chronic respiratory conditions to achieve increased levels of self-management and self-efficacy via mentoring support from community care nurses and technology supporting electronic self-monitoring techniques. Supporting electronic self-monitoring provides patients, nurses and clinicians with access to symptom and psycho-social data and contributes to assisting patients to improve the identification, comprehension and initiation of early action in relation to alterations in their conditions. The information systems approach is underpinned by the view that where possible patients should play a central role in decisions about their own health. This approach draws on the perspective that information is a form of care and that providing Cummings, Turner: Patient Empowerment and Self-efficacy in Chronic Disease Management 2 evidence-based knowledge to patients may positively enhance their ability to self-manage in the community and contribute to the development of an increasingly effective patientcentred healthcare system (Hill, 1998).
