- W2509064023 abstract "Dear Editor,We present an unusual presentation of an RF thermocouple electrode failure during a percutaneous cervical cordotomy on a man age 64 years with a four-month history of mesothelioma-related, right-sided chest wall pain. Following a joint palliative care–pain assessment, he consented to a left percutaneous cervical cordotomy and received standard peri-procedure care on our hospital’s cordotomy care pathway. Our unit is currently one of the two units in England offering a percutaneous cordotomy service, and has done for a number of years.The procedure was performed using our standard kit (MAX-T Disposable RF Cordotomy Thermocouple Electrode with adjustable length, GABI-N intermediate connecting cable, Minta Medical Ltd., UK) and the Neurotherm NT1000 RF generator. After having positioned the patient’s head in a cordotomy frame, the spinal needle (PAJUNK Quincke cannula 20 G X 80 mm) was inserted between the first and the second cervical vertebrae on the left side. Free flow of CSF was established, a myelogram was obtained, dentate ligament was identified, and the cordotomy electrode was passed just anterior to the dentate ligament, through the spinal needle into the anterolateral quadrant of spinal cord. Sensory and motor discrimination helped us confirm good position of the cordotomy electrode tip to be in the anterior spino-thalamic tract, and an initial RF lesion at 70°C for 25 seconds was initiated in automatic thermal lesion mode.
