Lawrence1* 1 Broadview Spine and Health Centre, Canada Background: A 45 year old female presented with chief complaints of headaches,poor concentration,disequilibreum and poor short term memory subsequent to head trauma four years earlier.Medical history revealed a series of four concussions prior to this event.A course of cranio sacral therapy,chiropractic and massage therapy did not attenuate her symptoms.These symptoms have prevented her from engaging in an exercise program as well as limiting full productivity at work. Methods: Neurological examination revealed right head roll,percussive myotonia for the left and right thenar eminence,decreased right arm swing during gait and right tongue protrusion.Videonystagmography(VNG) revealed right beating nystagmus in the dark,saccadic intrusions of 42%(N<18%) for horizontal smooth pursuit eye movement(HSPEM),saccadic latencies for left and right targets respectively were 171 and 202 msec(N<200msec),balance scores with eyes closed on hard and soft surfaces were 86% and 72%(mild reduction).This patient was prescribed eye movement exercises and vestibular rehabilitation exercises as well as somatosensory evoked potential(SSEP) stimulation of the tongue. Results: After sixteen treatment sessions she demonstrated neutral head posture and tongue protrusion,negative percussive myotonia,normal right arm swing,absent nystagmus in the dark,saccadic intrusions of 12% for HSPEM,saccadic latencies of 232 and 242 msec for left and right targets respectively,comparative balance scores of 93% and 78%(healthy scores).She was able to resume an exercise program and returned to full productivity at work. Conclusion: The author suggests further investigation into eye movement,vestibular and SSEP rehabilitation strategies for the treatment of post concussion syndrome.
