- W2511358468 abstract "Helicobacter pylori（H. pylori）感染診断法の1つである組織鏡検法は，非透析患者において頻用されている。しかし，透析患者は血栓予防のため低用量アスピリン，抗血小板薬を内服しており非侵襲的な診断法が望ましい。そこで，透析患者のH. pylori感染診断に13C-尿素呼気試験（Urea breath test : UBT）が有用かどうかを検討した。対象とした透析患者は27例，平均年齢66.8歳，平均透析期間3年1カ月である。非透析成人におけるユービットを用いた13C-UBTのcut-off値は2.5‰（20分値）である。H. pylori陽性透析例（10例）のΔ13CO2 20分値は透析日58.9‰，翌日40.6‰，翌々日58.3‰，H. pylori陰性例（17例）のそれは4.0‰，6.2‰，8.5‰であった。透析日の感度は90.0％，特異度は58.8％，一致率は70.4％，翌日の感度は100.0％，特異度は70.6％，一致率は81.5％，翌々日の感度は100.0％，特異度は70.6％，一致率は81.5％を示し，特異度，一致率が低かった。これは，H. pylori陰性例において，ウレアーゼ活性を有する胃内常在菌により，13C-UBT開始直後のΔ13CO2濃度が急上昇するためと考えられた。更に，H. pylori陽性例のΔ13CO2濃度低値例の存在も影響していると思われた。そこで，H. pylori陽性例，陰性例のΔ13CO2濃度オーバーラップが比較的少ない値より，透析患者に有用な13C-UBT cut-off値を算出した。その結果，透析日の13C-UBT開始10分後のΔ13CO2濃度に基づくreceiver operating characteristic（ROC）曲線より，cut-off値は5.0‰となった。非透析成人のcut-off値（2.5‰）を用いるよりも感度，特異度，一致率は良好であったが（それぞれ90.0％，88.2％，88.9％），組織鏡検法に比し劣っていた。" @default.
