- W2514444893 abstract "Advanced glycation end-products (AGEs) play a pivotal pathogenic role in ageing and diabetes. With IRB approval and informed consents, we showed adverse effects of AGE accumulation on ART outcomes in study I and in study II we improved ART outcomes in aged patients with repeated ART failures by reducing AGEs with Hishi-extract, anti-glycation functional food, which suppresses AGE production and also break down AGEs. retrospective analyses and prospective clinical trial (UMIN000017758). Study I: Toxic AGE (TAGE), pentosidine (Pent), and carboxymethyl lysine (CML) in blood and follicular fluid (FF) were measured in 157 ART-patients. We analyzed associations of AGE with ART outcomes. Study II: Thirty-two old patients (mean age of 42.2) with repeated (>3) ART failures received daily 100mg of Hishi-extract (Hayashikane) before and throughout ART. AGEs were measured before and 1-2 months after Hishi-extract. Study I: TAGE, Pent, and CML in FF, and TAGE in serum showed significant negative correlations with follicular and embryonic development. Women with serum TAGE above 7.24 U/mL showed decreased oocyte numbers and ongoing pregnancy rates (%OP), even with younger age or lower day-3 FSH; %OPs with high vs. low TAGE were 4.8% vs. 38% in 40>age>35 years (P<0.05) and 6.1% vs. 25% in day-3-FSH <10 IU/L (P<0.05), respectively. Study II: Embryonic developments were improved, unchanged and worsened in 75, 16 and 9% of 32 patients, respectively, compared with previous own ART without Hishi-extract. Seven clinical pregnancies (22% per patient) were achieved, resulting in 6 deliveries (19% per patient), which is significantly (p<0.001) higher than 3.6% in 838 ART cycles with age of 42 from the overall 2012 data in Japan. Hishi-extract significantly decreased TAGE in serum from 9.0±0.7 to 7.1±0.7. Serum CML significantly decreased with Hishi-extract in 7 patients with pretreatment CML >1.0 (high CML group), but not in 8 patients with pretreatment CML ≤1.0 (low CML group). There was a tendency for higher rate of clinical pregnancy by ART with Hishi-extract in the high CML group than in the low CML group (43% vs. 0%, p = 0.08). Serum TAGE and FF Pent accumulations correlate highly with poor follicular and embryonic developments and with unlikelihood of ongoing pregnancy. Serum TAGE is a novel, earlier marker to detect diminished ovarian reserve independently of age and day-3 FSH. Administration of Hishi-extract decreases AGEs and improves ART pregnancy rates dramatically in old patients with repeated failures. Its effects on reduction of AGE and improvement of pregnancy rate appear to be greater in patients with higher AGE. Detection and reduction of AGE accumulation are novel strategies for infertility treatment." @default.
