• W2519438674 abstract "The current pandemic of obesity and type 2 diabetes (T2D) emphasizes the need for novel techniques for evaluating human fatty heart in vivo. While fatty heart was recognized as early as the seventeenth century, recent clinical research using cardiac imaging has documented left ventricular hypertrophy and impaired systolic and diastolic function in morbidly obese humans. Biomedical research in rodent models of obesity and T2D has established the presence of high concentrations of triglyceride droplets within cardiomyocytes – cardiac steatosis – associated with contractile myocardial dysfunction, termed ‘lipotoxic dysfunction’. Remarkably, the treatment of rodents with antisteatotic agents prevented development of cardiac steatosis and preserved cardiac function, suggesting a causal link. The translation of these findings from bench to clinic, to evaluate human fatty heart in vivo, requires a noninvasive technique, and localized proton magnetic resonance spectroscopy (1H MRS) has emerged as a novel in vivo technique well suited to the task. Translational research on the human fatty heart using 1H MRS has confirmed the existence of cardiac steatosis and lipotoxic dysfunction in obese and T2D patients. Research in humans has shown that cardiac steatosis measured by 1H MRS may predict myocardial dysfunction and has the potential to become a biomarker for lipotoxic cardiac dysfunction in clinical trials. This article describes the cardiac 1H MRS technology required for human studies, reviews the present status of the field, and concludes with a look ahead to future clinical applications." @default.
