- W2521528051 abstract "Introduction Arterial BP has a linear relationship with cardiovascular risk, even within the “normal” BP range. Women with an HDP have increased future cardiovascular risk. The role of BP in this risk is unknown and may be under-estimated by reference to usual “normal” BP ranges. Objectives To evaluate post-partum 24 h ambulatory BP (ABPM), standard sphygmomanometric and central (aortic) BP in women with prior normal pregnancy (NP) or complicated by a hypertensive disorder of pregnancy (HDP). Methods Prospective cohort of women recruited to the ongoing P4 (Postpartum Physiology, Psychology and Paediatric follow up) study at St George Hospital, Kogarah, Sydney. Six months after either NP or pregnancy complicated by gestational HT or pre-eclampsia (HDP group) women were assessed by ABPM and pulse wave tonometry. Women with known chronic HT were excluded. Results In the 176 women recruited to date ( n = 136 NP, n = 40 HDP), at six months post-partum, 24 h ABPM BP was higher in the HDP group compared with NP (114 ± 7/71 ± 6 v 106 ± 7/66 ± 5 mmHg, p p p p 2SD above the NP mean, compared with 10% of NP women, p Conclusions Six months post-partum, women with HDP had higher BP compared to women with NP, confirmed by ABPM, which was also apparent at booking in the first trimester. We speculate that the increased BP is not caused by the pregnancy but pre-existing in woman who develop HDP, and perhaps carried through life as a cardiovascular risk factor." @default.
