The diverse backgrounds of the participants provided an ideal forum for discussing the three core questions posed by the conference organizers: (1) What is wasting? (2) What are the optimal methods to determine and track changes in body composition in large clinical trials? and (3) What endpoints should be used to demonstrate efficacy for products used to treat wasting? While there are many inherent differences in the patients and problems faced by these disciplines, we found common themes in that we both deal primarily with chronic rather than acute diseases, and that similar issues affect decisions regarding selection of endpoints for evaluating the treatment of wasting syndromes. Gastroenterologists have been stimulated to reconsider wasting syndromes by the AIDS epidemic and effects of cancer and chemotherapy (topics considered separately at this meeting), in addition to their traditional concerns about diagnosing and treating intestinal, pancreatic and hepatic disorders that interfere with the digestion, absorption, and assimilation of dietary nutrients. The recent appreciation that hypoalbuminemia is the most powerful predictor of mortality in end-stage renal disease highlights the critical importance of ensuring adequate protein intake in this patient population. Despite dialysis therapy, unfortunately, anorexia remains a pervasive problem, and alterations in protein turnover induced by dialysis and concomitant illness (e.g., metabolic acidosis, diabetes mellitus) further compromise the nutritional status of these individuals. Our discussion group drew heavily from the presentations given during the day and one-half conference preceding, particularly regarding the limitations of currently available methods for measuring body composition and selection of endpoints for clinical trial in different patient populations. We relied particularly upon Dr. Morey Haymond’s presentation on the selection of “Nutritional and Metabolic Endpoints.” With respect to the three core questions posed by the workshop organizers, we arrived at the following conclusions:" @default.
