Nous rapportons le cas d’une intoxication médicamenteuse volontaire à l’amlodipine et l’olmésartan chez une patiente de 63 ans en association avec des psychotropes. La prise en charge en réanimation a compris une circulation extra-corporelle, une hémofiltration et une épuration extrahépatique, ainsi qu’une administration de catécholamines et des traitements spécifiques (glucagon, chlorure de calcium, intralipides et insuline). La concentration plasmatique d’amlodipine à H2 (2 heures après admission) était de 373 μg/L. Son suivi toxicocinétique a permis d’observer une phase de décroissance suivie d’une ré-augmentation à H36 qui a motivé une fibro-endoscopie œso-gastro-duodénale (FOGD) qui a dévoilé un pharmacobézoard très volumineux. Malgré l’évacuation de ce dernier, la patiente est décédée au bout de 6 jours avec une concentration plasmatique d’amlodipine en franche diminution (55 μg/L) et une phase de relative stabilisation clinique. La présence de signes d’ischiémie digestive et des plages de nécroses cutanées importantes pourraient être expliqués par l’état de choc initial et le bézoard. Les concentrations d’olmésartan ont été mesurées a posteriori à 12,2 mg/L (H2) puis décrivent une ascension progressive pendant 5 jours jusqu’à 228,4 mg/L (H90). Cette évolution inhabituelle avec accumulation provient probablement d’une saturation des voies d’élimination. Ce cas décrit pour la première fois le suivi toxicocinétique d’une intoxication conjointe à l’amlodipine et l’olmésartan et renforce son intérêt pour la réalisation précoce d’une FOGD afin de rechercher et d’évacuer un potentiel pharmacobézoard. Though calcium blocker intoxication gravity is well described, angiotensin II AT1 receptor blocker intoxication is poorly documented. We report the case of a self-induced intoxication of a 63-year-old woman involving amlodipin, olmesartan in association with psychotropes. Intensive unit cares included an extra-body circulation, hemofiltration, extrahepatic purification, catecholamines and specific treatments (glucagon, calcium chloride, intralipids and insulin). Amlodipin plasma concentration measured 2 hours after admission reached 373 μg/L. Toxicokinetics allowed to observe a first phase of decreasing concentrations followed by a second phase of increasing concentrations at H36 which motivated a gastric endoscopy revealing a massive pharmacobezoard. Despite its evacuation, the patient died 6 days after her intoxication with an amlodipin plasma concentration decreased to 55 μg/L and a relative clinical stabilization. The important signs of digestive ischemia and cutaneous necrosis could have been explained by the importance of the initial shock combined with the pharmacobezoard. Olmesartan concentrations were determined a posteriori: H2 concentration was 12.2 mg/L and gradually increased during five days, up to 228.4 mg/L at H90. This unusual evolution with accumulation may be due to the saturation of the metabolic and elimination pathways. This case describes, for the first time, the toxicokinetics of a combined intoxication with amlodipine and olmesartan and reiterates the primary importance to carry out a gastric endoscopy earlier in order to look for a pharmacobezoard." @default.
