- W2524037527 abstract "235 Objectives [18F]FDG brain PET is well established for differential diagnosis of dementia disorders and Parkinsonian syndromes. As cerebral glucose metabolism (CMRGlu) is in principle coupled to cerebral blood flow (CBF), MRI-based CBF measurement for instance by arterial spin labeling (ASL) might be able to replace [18F]FDG PET. This simultaneous PET/MRI study investigated this feature for the first time on a biomarker level. Methods 37 patients (67±9yrs, 16 female) with cognitive (n=28, MMSE=24±5) and/or movement (n=9) symptoms underwent simultaneous [18F]FDG brain PET/MRI (Siemens 3T mMR). [18F]FDG PET: 250MBq, 30-60min p.i.. MRI: Anatomical 3D T1-MPRAGE and ASL (PICORE Q2TIPS, 18 slices a 4mm in 64x64 matrix, TE/TI/TR=16/2400/3400ms, 55-60min p.i.). The [18F]FDG and ASL data were analyzed visually (image quality, binary judgment: normal/abnormal, pattern recognition) as well as relative quantitatively (signal intensity normalization: global): VOI-based (AAL template, PMOD) and voxel-based (SPM). Results All [18F]FDG images and 70% of the ASL images were visually judged as of appropriate quality for further analysis. Of the remaining image pairs, binary visual ASL analysis had an extrapolated specificity and specificity of 72% and 88%, respectively, as compared to [18F]FDG. Signal distribution pattern was matched between ASL and [18F]FDG in 65% of the cases. VOI-based analysis showed a positive correlation between rCBF and rCMRGlu in 29/70 cortical and subcortical VOIs, while the respective values were significantly lower for ASL as compared to [18F]FDG in 14/70 (mainly frontal, mid-cingular, and striatal) VOIs. SPM analysis confirmed these VOI results (see attached figure). Conclusions More work is needed to improve data quality of ASL MRI, especially to derive individual diagnostic quality. Then, ASL MRI might have the potential of providing [18F]FDG-like brain data, an opportunity which is of special interest for combined specific tracer PET/ASL MRI applications." @default.
