- W2533873180 abstract "Critically ill trauma patients present a range of particular challenges with respect to prevention and treatment of venous thromboembolism (VTE). The incidence of VTE is high in trauma patients. Prevention should be through combined mechanical and chemical methods unless there are contraindications. There is a small amount of evidence to indicate superiority of low molecular weight heparin over unfractionated heparin in the prevention of pulmonary embolism. Patients with head and spinal injuries are particularly at risk from VTE but it is extremely difficult to provide definitive guidance as to the timing of thromboprophylaxis initiation. VTE should be diagnosed using multiple modalities, including Duplex and cardiac ultrasound as well as CT. Patients with pulmonary embolism (PE) should have their risk stratified depending on the presence of absence of right ventricular (RV) strain. Those with RV strain should be considered for thrombolytic treatment or catheter embolectomy. The use of inferior vena caval filters may offer a solution to prevention of VTE in high risk patients, particularly those with contra indications to chemical thromboprophylaxis." @default.
