- W2538073505 abstract "Mounting evidence suggests a link between diabetes-related processes and increased risk of cognitive decline and dementia. Optimal disease self-management may be key in the prevention of cognitive decline among older diabetic patients, but may be compromised due to sub-clinical cognitive impairment. Although the benefits of computerized cognitive training (CCT) on cognition have been reported in persons with mild cognitive impairment and healthy older adults, whether CCT is beneficial for cognition and disease self-management in non-demented older diabetic adults is unknown. Objective: Our primary aim is to evaluate the effects of CCT on cognitive and disease-self management outcomes in non-demented older diabetic adults. Secondary aims are to study the effects of CCT on non-cognitive outcomes including mood, meta-cognition, self-efficacy, and caregiver burden, and to explore variables modulating treatment response. Following screening, participants (N=120) undergo baseline evaluation including cognitive and disease self-management measures. Participants are then randomized into a CCT or Active control (AC) conditions and train approx. 30 min., 3 times per-week for 8-weeks. Theory-informed behavior change techniques (BCTs), including self-efficacy management, self-monitoring and goal-setting are used in both groups to maximize adherence. Assessments are repeated after the intervention, and at a 6-month follow-up, with 1-week booster training completed 3 months from completion of the intervention. Study flow to date is depicted in Figure 1. Recruitment commenced in October 2015 and will continue to August 2016. Participants allocated to date to the CCT and AC shown no differences on sociodemographic or diabetes-related variables. To date, participants (16 males, 11 females) have a mean age of 72, and have completed 16.2 years of education. Several participants have reported difficulty (mostly technical) independently managing the training from their home. However, this had minimal effect on adherence and completion rates. This is the first RCT of CCT in older diabetic adults. Findings will clarify whether CCT has an impact on disease self-management and whether an association between cognitive function and disease self-management exists. The trial will inform future efforts aimed at improving disease self-management and cognitive functioning in this population, which may play a role in the prevention of dementia.
