- W2538379166 abstract "Alzheimer’s disease (AD) transforms our relationships with others. It corrodes our sense of self, distancing us from our former selves and in turn our relationship with friends and family. Technologies offer new possibilities of self-expansion and the construction of new relationships across geographical divides. Telemedicine for dementia care intervenes at the nexus of Alzheimer’s and new technology. To understand the transformative effect of telemedicine in dementia care we need a variety of research methods. While there have been several studies exploring the use of telemedicine for dementia care, ethnographic data to understand patient and clinician’s experiences is lacking. Ethnographic data is an essential complement to other methods for teledementia study to generate a better understanding of the nuances of patient, caregiver and clinician experience. Here we provide preliminary ethnographic data from our evaluation of patient, caregiver, and clinician experiences as part of a teledementia pilot study, Alzheimer’s Care via Telemedicine (ACT-ON), Phase 1. Data generated from six hours of semi structured interviews with 28 caregivers and patient dyads and 3 clinicians as well 22 recorded telemedicine visits were analyzed in order to generate a tableau of clinician and participant experience. Study dyads lived as far as over 200 miles from the test site. Patients had a range of disease severity from mild to advanced AD. The preliminary data suggest that routine clinical assessment of Alzheimer’s patients and caregivers over the teledementia platform can be done reliably and with high caregiver satisfaction. However a closer look at the experience of clinicians, caregivers and patients involved in the telemedicine study reveals some of the unexpected benefits as well as fissures that weaken the potential of telemedicine to replace traditional face-to-face clinical care. Close analysis of especially when a teledementia visit unravels reveals the nuanced nature of a successful telemedicine visit and the requirements of good design. Features for optimized teledementia interactions should emphasize a simple screen interface, adjustment of test administration to reflect the stage of dementia, and balance between care and assessment that reflects the needs of the patient and caregiver." @default.
