- W2555163802 abstract "Aims: To provide a review and analysis of the non-Australian literature on screening and mass treatment for bacterial sexually transmissible infections (STIs) for health services to consider in developing local programs. Methods: Systematic search of the English- language literature since 1985 via MEDLINE, with a more limited search back to 1970 and, in addition, hand searching of references cited in articles reviewed. Results: Various studies showed the feasibility of a broad range of strategies. Cost benefit studies demonstrated the utility of antenatal syphilis screening at very low prevalences of disease and of chlamydia screening in women at prevalences of 2 to 14 per cent. Chlamydia screening programs in America and Sweden involving millions of people over several years were followed by large falls in rates of infection. A randomised controlled trial of screening for chlamydia was followed by a substantially lower incidence of pelvic inflammatory disease. Mass treatment programs were associated with reduced rates of infection in treated groups and their sexual partners who were not treated. Conclusions: Taken individually, the lack of control groups and the influence of simultaneous education and condom promotion in most studies make it impossible to directly ascribe the reduction in infection to screening. However, the body of evidence strongly suggests that, if appropriately conducted, screening programs can have a positive effect, particularly for chlamydia.
