- W2558840106 abstract "Abstract Older adults have higher rates of diseases and conditions, take more medications, undergo more procedures, and consume more health-care dollars than any other age group. Despite their overrepresentation in every aspect of health care, they are underrepresented in every aspect of the basic and clinical research that informs etiology, diagnosis, and treatment. This underrepresentation is driven largely by the constraints imposed by traditional conceptual and pragmatic approaches to research. Traditional concepts underlying disease causation assume that there are cohesive underlying cellular and physiologic abnormalities that lead to specific diseases that are well defined in terms of organ localization, pathophysiology, and clinical features. Thus research seeks to reduce or eliminate potentially confounding factors such as coexisting disease. In parallel, biomedical research is constrained by multiple pragmatic issues of feasibility and safety that limit inclusion of older humans and animals. The recent explosion of knowledge about cellular and molecular mechanisms of aging and disease is changing long-held core concepts. It is increasingly likely that alterations in the same set of basic biological processes lead to both aging and susceptibility to multiple chronic diseases. In consequence, with aging, the abnormalities that lead to multiple coexisting diseases are likely to be due to shared, age-related, underlying molecular and cellular homeostatic processes. Through the creation of a multidisciplinary and multinational network under the rubric of “geroscience,” basic and clinical scientists are collaborating to build novel translational strategies to overcome prior conceptual and pragmatic barriers to the development of effective interventions to delay aging and chronic disease and prolong active life expectancy. This chapter presents an overview of issues related to preclinical and clinical aging research, describes new models of aging and disease causation, and addresses opportunities to refine research methods to accommodate the special challenges of aging research." @default.
