- W2559608764 abstract "As a nurse who has only worked in the emergency department ad who has been the manager of an emergency department for the past year, I truly enjoyed reading “Medical Mondays: ED Utilization for Medicaid Recipients Depends on the Day of the Week, Season, and Holidays” by Castner, Yin, Loomis, and Hewner.1Castner J Yin Y Loomis D Hewner S Medical Mondays: ED utilization for Medicaid recipients depends on the day of the week, season, and holidays.J Emerg Nurs. 2016; 42: 317-324Abstract Full Text Full Text PDF PubMed Scopus (22) Google Scholar The article brings very intriguing issues to the light. While it is no secret that patient volumes depend on the calendar day of the year, the day of the week, and the season of the year, I had not given much thought to separating out patients who do not have access to a plethora of resources. Regardless of the target population identified by each department, it is crucial to project census trends to appropriately allocate resources. Marcilio, Hajat, Gouveia, and Merchant2Marcilio I Hajat S Gouveia N Merchant RC Forecasting daily emergency department visits using calendar variables and ambient temperature readings.Acad Emerg Med. 2013; 20: 769-777Crossref PubMed Scopus (84) Google Scholar developed methods to forecast the daily number of patients presenting to the emergency department. Several methods were tested to determine the most accurate estimation tool. Two of the 3 models tested showed more accurate trends. Interestingly enough, the authors found that temperature did not significantly affect patient volumes in the emergency department, signifying that seasonal swings are likely due to a factor other than temperature. I suggest employing one of the aforementioned statistically significant models to calculate calendar trends in order to enhance the planning of and allocation of resources. This concept is worth understanding for nursing leadership. A way to correctly project ED volume by calendar day has been sought after for years because accurate projection of volume allows leadership to staff the best number of employees, prepare for departmental saturation accordingly, request that ancillary staff, such as case management and social work, be present at the right times, and ultimately provide patients with the best care that will optimize patient outcomes and prevent going over budget in the process. Medical Mondays: ED Utilization for Medicaid Recipients Depends on the Day of the Week, Season, and HolidaysJournal of Emergency NursingVol. 42Issue 4PreviewThe purpose of this study is to describe and explain the temporal and seasonal trends in ED utilization for a low-income population. Full-Text PDF Response to GautreauxJournal of Emergency NursingVol. 42Issue 6PreviewThank you for the opportunity to respond to Gautreaux ' s comments on our study investigating the calendar trends of ED volume. It is a noble goal to strive for a future where the patient arriving at the emergency department is met with the ideal match of skill mix, staffing, supplies and workflow to optimize his or her care, all within appropriate budgetary constraints. We agree the operational promise for time-series analysis, similar to what we used in our study, is to accurately forecasting patient volumes in a manner that will enable hospital leadership to optimize staffing and resources.
