Surgeries are controversial, and little evidence is available about outcomes. This retrospective cohort study describes survival and use of surgery among children with trisomy 13 and 18 born over a twenty-one year period in Ontario, Canada. We used linked health administrative databases to identify children born in Ontario between April 1, 1991 and March 31, 2012 with a diagnosis code for trisomy 13 or 18 on a hospitalization or same-day surgical record in the first year of life. We extracted all procedure codes through March 31, 2013. We calculated survival from birth and death dates; children living at end of follow-up were censored at the last clinical encounter. Our cohorts included 174 children with trisomy 13 and 254 children with trisomy 18. Median survival times were 9 days and 12.5 days, and one-year survival probabilities were 20% and 13% for children with trisomy 13 and 18, respectively. Children with trisomy 13 and 18 had ten-year survival of 13% and 10%. Children receiving surgical interventions, ranging from myringotomy to hemi-Fontan, accounted for 24% and 14% of the cohorts. One-year post-operative survival probabilities were 71% and 69%. Longer-term survival and surgeries were more frequent in this cohort than previously described. Although early mortality was common, more than half of the children who lived for five months survived ten years. Of the children who received surgical interventions, more than two-thirds survived longer than one year post-operatively. These conditions are not universally ‘lethal,' nor are all surgical interventions performed on children with trisomy 13 and 18 ‘futile.'" @default.
