Although these theories are well-accepted, we speculate that vision not well-restored after DSAEK may be due to the change of corneal structure and/or postoperatively induced cystoid macular edema (CME). However, the incidence of CME and the associated causative clinical factors in the post DSAEK have yet to be elucidated fully. We prospectively investigated the incidence of anatomic CME and its causative clinical factors post DSAEK via optical coherence tomography (OCT) and detailed retinal fundus examination. The authors describe an increased incidence of postoperative cystoid macular edema (CME) after Descemet stripping automated endothelial keratoplasty in association with possible causative clinical factors. Their finding of an increased rate of CME in a subgroup of glaucoma patients with multiple surgeries in their medical history led them to the conclusion of a possible inflammatory cause of CME.
