Two aspects are discussed: nanoparticles as contrast agents and imaging nanoparticles to get more insight into their optimization. The conception of nanoparticles as contrast agents to enhance the sensitivity of widespread imaging techniques led to commercially available nanoparticles such as iron oxide nanoparticles for magnetic resonance imaging and microbubbles for ultrasound imaging. A number of nanoparticles as contrast agents are currently developed for optical imaging techniques as well. Following nanoparticles by the means of imaging techniques allows gathering crucial information on the nanoparticle fate and clearance within the body. Size, surface charge, and coating stability are getting constantly optimized and should lead to advances in the pharmacokinetic properties required to bring nanoparticles to the health market. The current clinical applications of nanoparticles, described in the third part of this chapter, exploit the uptake of nanoparticles by macrophages to image macrophage-related disease. Nanoparticles can be found in a number of clinical trials related to macrophage imaging such as lymph node imaging, detection of rejected transplant, or infiltration of macrophages in diabetes or cancer. They might also have a role to play as blood pool agents or specifically targeted cell subtypes as will be discussed further in the chapter.
