- W2566515280 abstract "Department of Nephrology, Free University Berlin,Berlin, GermanyA widespread, rather general, definition of contrast-inducednephropathy (CIN) is an impairment in renal functionoccurring within 3 days following the intravascularadministration of contrast media (CM) and the absence of analternative aetiology. In spite of the vast clinical importanceof CIN, its understanding and the pathophysiology behindCIN remain incomplete. Many studies have been performed;however, they have provided no widely accepted conclusionso far. Here the possible mechanisms underlying CIN areoutlined, which span from altered rheological properties,perturbation of renal haemodynamics, regional hypoxia,auto-, and paracrine factors (adenosine, endothelin, reactiveoxygen species) to direct cytotoxic effects. Although thesepotential mediators of CIN will be discussed separately,several factors may act in concert to perturb kidney functionafter exposure to contrast media. From the currentknowledge of the mechanisms causing CIN, it is not possibleto recommend a certain class of contrast media, except toavoid large doses of CM of the first generation. From apathophysiological perspective, volume expansion iseffective in avoiding CIN, since water permeability of thecollecting ducts will decrease and enhance fluid excretion.Hence, CM in the distal portions of the tubular system isdiluted, which implies reduced fluid viscosity and a lower riskof obstruction." @default.
