- W2570759916 abstract "OBJECTIVE: Determine the relationship between gray matter (GM) volumetric and resting-state functional connectivity (rsFC) abnormalities in temporal lobe epilepsy (TLE). Assess the ability of these measures to explain episodic memory (EM) deficits.BACKGROUND: TLE patients suffer from EM deficits. To date, there are no studies combining structural GM and rsFC measures to explain these cognitive deficits in TLE.DESIGN/METHODS: 85 TLE patients (47 left (23 mesial temporal sclerosis (MTS)), 38 right (15 MTS)) and 51 controls realized an anatomical MRI scan (T1-MPRAGE) and a five-minute resting-state functional MRI scan. EM skills were assessed using neuropsychological testing (verbal: Wechsler Memory Scale (WMS) Logical Memory (LM) I and II, CVLT; non-verbal: WMS Facial Memory (FM) I and II). Individual whole-brain GM probability maps were computed using Voxel-Based Morphometry (VBM8 toolbox) and compared between the groups. RsFCs were computed between the regions showing abnormal GM volume and compared between the groups. Lastly, we utilized correlation analyses to test whether rsFC or GM probability values from these regions were associated with EM performance.RESULTS: VBM analyses revealed TLE groups possessed reduced GM volume in the epileptogenic hippocampus, as well as the superior medial frontal cortex, bilateral cerebellum and left postcentral gyrus. RsFC analyses revealed no significant differences between TLE and controls. Verbal EM was positively correlated with left hippocampal GM, and negatively correlated with rsFC between left-hemisphere regions, in left TLE. Non-verbal EM was positively associated with right temporal GM, and negatively correlated with rsFC between right-hemisphere regions, in right TLE.CONCLUSIONS: This multi-modal study provides evidence of a positive relation between GM loss and EM deficits in TLE patients, as a function of EM material and the pathology side. In contrast, the rsFC between these regions had a negative impact on EM skills, suggesting potential functional maladaptive changes in the pathologic hemisphere.
