At one stage in my career during the 1980s, I was a trauma surgeon, operating on patients with stab and gunshot wounds in Johannesburg's Soweto township in South Africa. I did a lot of sewing then—bowel resections, vascular anastomoses, repairs, and reconstructions—and I thought I was good at it. Synonymous with high quality, and often shorthand for expensive, the obvious focus of bespoke is its outcome—a suit, say, or a jacket. To Joshua, however, bespoke is more than a product or even a process. Bespoke for him is a philosophy, encapsulating not only how he does his work but also the principles that underpin it. Full-Text PDF Dissecting the consultationAs a medical student, I saw medicine as a science. As a surgeon, it was about skills. As a general practitioner (GP), medicine was a performance. As a patient, I recognise all of these, although what I experience most directly is the performance. Underpinning the science of medicine and the skills of surgery is the consultation.
