- W2578377964 abstract "646 Objectives: Aim of our study was to assess the accuracy of 18F FDG PET/CT in staging ovarian cancer, using as a standard of reference surgical and histological stage. Methods: Our population consisted of 29 patients with ovarian cancer already scheduled for surgery (age: mean 66; range: 23-89). All patients underwent a FDG PET/CT and a CT within 2 weeks before surgery. We performed standard FDG PET/CT scan. FDG PET/CT images were analysed visually and SUVmax was measured in the site of the primary tumour. PET/CT and CT stage followed FIGO criteria and were compared to surgical, histological and final staging after a follow up period of at least 6 months. Results: Final staging revealed 9 patients in stage I (5 Ia; 2 Ib; 2 Ic); 3 in stage II (1 IIa; 2 IIc);12 in stage III (2 IIIa; 5 IIIb; 5 IIIc) and 5 in stage IV. PET/CT results were concordant with final diagnosis in 21/29 patients: six out of nine patients in stage I; one out of three patients in stage II; ten out of twelve patients in stage III and four out of five patients in stage IV. In one patient, an unknown synchronous breast tumour was correctly detected by PET/CT only. PET/CT was completely false negative in two stage I patients, where a micro invasion of a serous papillary adenocarcinoma was histologically detected; moreover PET/CT incorrectly down staged five patients (3 to stage Ia from stage Ic, IIc, IIIa; one to stage IIIa from stage IIIc and one to stage IIIb from stage IV missing two small liver metastasis). PET/CT was falsely positive only in one patient showing increased FDG uptake in inguinal lymph nodes that turned out to be reactive at histological analysis. CT results were concordant with final diagnosis in 14/29 patients. It is interesting to point out that CT incorrectly down staged 4 out of 5 stage IV patients, missing distant metastasis in the pleura, in mediastinum and in left clavear lymph nodes, that were correctly detected by PET/CT. Overall, FDG PET/CT showed a better accuracy than CT (73% vs 48%) in staging ovarian cancer. Conclusions: this study suggests that PET/CT with FDG can increase the diagnostic accuracy of pre surgical staging of ovarian cancer in comparison with Computed Tomography. The correct distinction of stage IIIb from IIIc or IV, can address towards a neo adjuvant therapy rather than a surgical approach." @default.
