- W2582801798 abstract "Introduction: Since 20-year follow-up results of two pioneering randomized controlled trials demonstrating equal survival after mastectomy and breast conservation therapy, breast conservation therapy undoubtedly represents substantial progress for a better quality of life for women with breast cancer. The outcome of breast conservation therapy at Seoul Asan Medical Center were retrospectively reviewed, and analyses carried out to obtain the 5 year disease free survival rates and significant prognostic factors. Methods: Between January 1997 and December 2002, 626 women with stage I, II, III breast cancer were treated with conservative surgery and radiation therapy. The outcomes of local recurrence and survival were recorded. The 5 year survival rates were obtained using the Kaplan-Meier method and the prognostic factors were studied with univariate and multivariate analyses using SPSS 11.0. Results: The average age at the time of operation was 45.5 years(18-82 years), and the mean tumor size was 1.7cm(0.1-8.0cm). Four hundreds and forty four(70.9%) had pathologically node negative disease. During a median follow up of 50.0 months, 36(5.8%) developed locoregional recurrence only and 24(3.8%) developed systemic recurrences. The 5 year disease free survivals were 88.9%. The univariate analysis of the prognostic factors showed that axillary nodal status (p=0.00), presence of lymphovascular invasion(p=0.00), surgical resection margins status(p<0.01), estrogen receptor positivity(p<0.01), progesterone receptor positivity(p=0.00) and high nuclear grade (p=0.01) were statistically significantly associated with locoregional recurrence only. The results of the multivariate analysis revealed that the axillary nodal status(p=0.00) and progesterone receptor positivity(p<0.04) were strongly independent predictors for locoregional recurrence only. Conclusions: A breast conserving procedure is useful and effective for patients with invasive breast cancer, and the axillary nodal status was the strongest independent predictor for the locoregional recurrence." @default.
