- W2584931612 abstract "Background: Oocyte cryopreservation has gained recent interest as a potential means of preserving fertility. The protocol for this technique continues to be modified to achieve a high success rate. Objective: To compare the clinical outcome following oocyte cryopreservation using a GnRH agonist versus antagonist stimulation protocol. Materials and Methods: Eighteen women ≤ 35 years of age with tubal factor infertility underwent pituitary suppression using a GnRH agonist (n=10) or antagonist (ganarelix acetate) (n=8) and HMG (Repronex, Ferring Pharmaceuticals Inc., Albuquerque, NM) for ovarian hyperstimulation. Oocytes were stripped of the cumulus cell mass and cryopreserved using a sodium-depleted HEPES-HTF based medium containing amino acids with the cryoprotectants propanediol (1.5M) and sucrose (0.3M) in a standard slow-cooling protocol. Immature oocytes were cultured overnight and cryopreserved the following day if a polar body was present. After a one-month rest cycle, subjects underwent endometrial preparation with oral estradiol and vaginally-administered micronized progesterone. Oocytes were thawed using a standard rapid warming procedure and transferred to sequentially lower concentration sucrose in sodium-depleted HEPES-HTF medium. Surviving oocytes were incubated in culture medium for 3-4 hours before undergoing ICSI. Up to 4 cleaving embryos were assisted hatched using acidified Tyrode’s Solution 3 days later and transferred to the uterus trans-cervically. Results: Ten women assigned to GnRH agonist underwent 12 ET cycles and 8 women undergoing antagonist cycles underwent 8 ET cycles.
