- W2586933612 abstract "Background: Pancreatic ischemia/reperfusion injury (IRI) is an unsolved problem in pancreas transplantation. Amelioration of the IRS is a central point to reduce morbidity after pan creas transplantation. Methods: We investigated in a model of normothermic in-situ IRI of a pancrea tic tail segment, pedunculatet on the splenic vessels, the outcome after 2 h of ischemia and 2 h of reperfusion at different ischemia temperatures (4 °C, 18 °C and 37 °C). Parameters were intravital fluorescence microscopically (1 h and 2 h after reperfusion) measured func tional capillary density (FCD [cm/cm 2 ]), the number of adherent leukocytes (LAV [cells/mm 2 ]) and histological damage (score: edema, vacuolisation, PMNinfiltration and necrosis; SHS [score points]). Results: FKD: Decrease was the lowest at 4 °C ischemia 1 h (–12,4 %) and 2 h (–14.9 %) after reperfusion (18 °C: –23.6 % and 25.8 %; 37 °C: –20.2 % and –23.6 %). Compared to the other temperture groups, microcirculation was significantly better in 4 °C ischemia after reperfusion: (1 h: 380.5 cm/cm 2 ± 5 SEM; 2 h: 369.5 ± 7; 18 °C: 339.5 ± 2 and 329.4 ± 5, p < 0.001; 37 °C: 348.4 ± 5 and 333.5 ± 6, p < 0.01), there was no diffe rence between 18 °C and 37 °C. LAV: 2 h after reperfusion the number of adherent leu kocytes was lowest at 4 °C ischemia (41.1 cells/mm 2 ± 24.0 SEM; 18 °C: 63.4 ± 18.5, p < 0.05; 37 °C: 282.9 ± 46.7, p < 0.01). HS: Histological damage was significantly reduced in 4 °C ischemia (5.0 score points ± 0.5 SD; 18 °C: 6.6 ± 0.4, p < 0.05 and 37 °C: (9.1 ± 0.3, p < 0.01). Conclusions: Ischemia temperture has a significant influence on postischemic pancreatic micro circulation, inflammatory response and histological damage. In pancreatic ischemia, tempreature of 4 °C shows better protection than 18 °C or 37 °C. Probable reasons here could be organspecific factors of the pancreas, e. g. decreasing of activity of pancreatic enzymes and reduced metabolic activity with enhanced recovery after reperfusion. This experimental data show that preservation at 4 °C shows actually the best protection of the organ before transplantation. Einleitung" @default.
