• W2588690661 abstract "Introduction: Cardiac MR (CMR) is the reference standard for assessing macroscopic myocardial scar. This technique relies on maximizing contrast between normal and abnormal myocardial segments, with limitations in diffuse fibrosis. Determination of the extracellular volume fraction (ECV) by T1 mapping quantitates the extent of diffuse left ventricular (LV) fibrosis. ECV application to the thin, trabeculated right ventricle (RV) is challenging, however, the purpose of this study is to evaluate an optimized high-resolution (HR) T1 mapping technique at 3T for RV ECV calculation in healthy controls. Methods: Retrospective evaluation of data from 20 healthy volunteers who underwent CMR at 3T (Magnetom Skyra, Siemens Medical Systems). T1 mapping was performed in the axial orientation using a HR fast-MOLLI technique, with an in-line motion correction algorithm enabling automated T1 and T1* map generation with a 1 x 1 x 8 mm3 voxel. The MOLLI sequence was comprised of two inversion pulses with single shot steady state diastolic readouts. The T1 recovery was sampled over 5 and 3 heartbeats, for the two inversion pulses. Three recovery heartbeats were included before the second inversion pulse for T1 regrowth. Images were acquired pre- and between 10 and 25 minutes post-contrast (0.2 mmol/kg gadobenate dimeglumine). A single reviewer placed ROIs on the basal and mid RV, interventricular septum, and lateral LV wall on T1 maps; RV blood pool signal intensity was obtained on T1* maps. ECV was calculated from normal hematocrit values (0.38 to 0.46 for women, 0.42 to 0.54 for men). Average LV and RV ECV values were compared using the student's t-test. Intraobserver variance was measured by the intraclass correlation coefficient (ICC). Results: T1 mapping delineated RV contours enabling ROI placement in all subjects. The RV and LV ECV values were statistically different, ranging from 27.3 to 34.0% and 22.1 to 27.6% for the lowest and highest extreme normal hematocrit values respectively (p < 0.0001). The ICCs for the global RV and LV were 0.67 and 0.70 respectively.
