- W2595210147 abstract "医療材料の内外価格差については広く取り上げられてきた一方で，欧米で広範に使用されている医療機器が日本では同頻度で使用されていないという批判もある。この理由は必ずしも貿易障壁があるということを意味しない。もちろん，わが国の事情として，医療機器の審査に時間がかかり，ディバイス・ラグが生じているという事実は否めないが，国により疾病構造に差異があり，同じ疾病の予後も異なっているためにあまり使用されていない場合がある。その代表が虚血性心疾患である。虚血性心疾患の頻度が欧米に比して少ないことや，欧米の心筋梗塞の予後とわが国のそれとを比較した場合，わが国の方が予後がよいことが多くの研究で知られている。急性心筋梗塞の米国のNRMIレジストリーと東京女子医大関連病院グループでのHIJAMIレジストリーを比較すると，前者のLVEF40％未満が全体の23％を占めるのに対して後者は17.1％であった。次に，わが国の心臓突然死についてもShigaらの研究のKaplan-Meierカーブによれば，米国のそれよりも予後がかなりよかった。さらに，われわれのレジストリーを用いて，わが国における心臓突然死予防のためのICD植込みの費用対効果を試算してみた。すなわち，先行研究であるSCD-HeFT研究の基準を満たした172例の70％がICD植込みにより突然死を免れたと仮定して費用対効果を計算したところ，10,048,000円/LYSとなり，米国のSandersらによるメタ解析の結果と比べ劣っていた。" @default.
