- W2597014254 abstract "This is a retrospective study of the impact of stereotactic radiation in five fractions on the thoracic aorta and cardiovascular health of patients treated for lung cancer. All patients treated at a single institution between January 2006 and December 2013 with stereotactic ablative radiation to the lung were identified, and those treated with 5 fraction treatments with at least 1 follow up scan were selected for this study. The medical history, comorbidities, and treatment for cardiovascular disease including hypertension were determined. The anterior-posterior, and lateral diameters of the aorta were determined for pre-treatment and post-treatment scans. The aorta was examined both as a solid structure as well the outer wall of the aorta. The dose received by the aorta and aortic wall was determined to the 5 cc, 2 cc, 1 cc, and 0.03 cc volume. The Generalized Estimating Equation was used to determine if there was a significant change in diameter over time and if a change in diameter was associated with the dose received. Twenty three patients were identified that were treated with 5 fractions and had at least one follow up scan. The follow up ranged from 5 months to 5 years with a median of 2 years. Nineteen patients had hypertension that was treated with medications prior to treatment. Five patients had no change in there antihypertensive medications, five had an increase, and five had a decrease in medications while four had a change in medication that could not be classified as an increase or decrease in medications. Four patients had a previous history of cardiac disease. One patient had a myocardial infarction fifteen months after radiation treatments. There were no developments of No patient developed an aortic aneurysm during the follow up period. The point dose to the aortic wall (0.03 cc) ranged from 5.4 Gy to 72.8 Gy with a median of 29.7 Gy. There was no significant decrease in the diameter or area of the aorta from before treatment to after treatment. However, the dose received by the aorta and the aortic wall to 5 cc, 2 cc, 1 cc, and 0.03 cc were all correlated with a decrease in aortic diameter over time, but this change was 0.07 mm short axis per Gy per year to the 0.03 cc aortic rim volume. Aortic dose received during stereotactic radiation is associated with a decrease in aortic diameter, but the decrease in aortic diameter was not associated with a significant change in cardiovascular risks such as hypertension or a significant increase in cardiac events. Without changes in cardiac risk factors, the change in aortic diameter is of questionable clinical relevance in stereotactic radiation." @default.
