Gerec ve yontem: Klinigimize diz agrisi, dizde bosalma, kilitlenme yakinmasi veya travma oykusu nedeniyle ortopedi veya fizik tedavi rehabilitasyon doktoru tarafindan, diz magnetik rezonans goruntuleme (MRG) incelemesi yapilmak uzere basvuran 118 hasta medial ve lateral meniskus, medial kollateral ligaman (MCL) ve lateral kollateral ligaman (LCL) ve anterior krusiat ligaman (ACL) ve posterior krusiat ligaman (PCL) lezyonu bakimindan degerlendirildi Bulgular: Medial meniskus, MCL ve ACL lezyonu uclusu 4 olguda (% 3.4), lateral meniskus, MCL ve ACL lezyonu uclusu 2 olguda (% 1.7) bulundu. ACL ve MCL kombine yirtigi 4 hastada (%3.4) bulundu. Sonuc: ACL, MCL ve medial meniskus yaralanmasi kombinasyonu olan O.Donoghue.nin mutsuz triadi, ACL, MCL ve lateral meniskus yaralanmasi triadina gore daha sik bulundu. MRG.nin klinik kullanima gecmedigi donemlerde hangi mutsuz triadin daha sik goruldugune dair yapilmis karsit calismalar yapilmistir,bunlarin hepsi artroskopi ve cerrahi yontemlere dayanmaktadir. O donemlerde meniskus ve bag yaralanmalarinin preoperatif degerlendirmesi sadece fizik muayene ile yapildigindan, cerrahi karari verilmesi bakimindan bu tur klinik degerlendirmeler yapmak cok onemli idi. MRG, konservatif veya cerrahi tedavi kararinin verilmesinde guvenilir, kostefektif, sensitif ve spesifik bir goruntuleme yontemi oldugundan eklem ici yapilarin preoperatif degerlendirilmesinde cok onemli rol oynamaktadir. Abstact The examine of the frequency of combined injuries of knee joints clinically known as the classical unhappy triad by magnetic resonance imaging Purpose: The aim is to examine the frequency of combined injuries of knee joints clinically known as the classical .unhappy triad. by magnetic resonance imaging (MRI). Matherial and methods: 118 patients, who referred to our clinic for knee examination to be performed by an orthopedist or physical therapy and rehabilitation physician by magnetic resonance imaging due to knee pain, giving way of knee, locking complaints or trauma history, were evaluated in terms of medial and lateral meniscus, medial collateral ligament (MCL) and lateral collateral ligament (LCL) and anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) lesions. Results: The triple combination of medial meniscus, MCL and ACL lesionswas found in 4 cases (3.4%) and that of lateral meniscus, MCL and ACL lesions was found in 2 cases (1.7%). Combined ACL and MCL tear was found in 4 patients (3.4%). Conclusion: O.Donoghue.s unhappy triad, which is the combination of ACL, MCL and medial meniscus injury, was found more frequent than triad of lateral meniscus injury. Some contrary studies were done so as to determine which unhappy triad was more frequent during the time before MRI was clinically used, and all these studies were based on arthroscopy and surgical methods. As preoperative evaluation of meniscus and ligament injuries was only made by physical examination in those periods, performing such clinical evaluations was crucial in terms of the surgical decision. Being a reliable, cost-effective, sensitive and specific imaging method for conservative or surgical treatment decision, MRI plays a very important role in preoperative evaluation of inner joint structures.
