- W2599544462 abstract "The discussion of psychological outcomes after a burn injury began with a review of the research findings during the past two decades. This discussion was followed by a focus group of burn professionals and burn survivors that was tasked with building on the information from the past decade and identifying research priorities for the next 10 years. The scientific progression of psychological outcomes in burn survivors during the past two decades has been based on a series of different models that build on each other. We first started with a medical model that was focused mostly on survival, with an emphasis on resuscitation and infection control. Psychological research within this model emphasized pain control, with groundbreaking studies on the undermedication of pain,1,–3 the importance of managing anxiety as well as pain,4 and nonpharmacological interventions for pain and anxiety.5 As more people survived, we began to look at rates of pathology and progressed to a psychopathology model as we began to acknowledge that the nature of the trauma, the painful treatments, and the disruption of social roles could cause psychological disorders, such as anxiety, depression, and posttraumatic stress disorder (PTSD).6,–8 At this time, we also saw the development of large databases that allowed us to track incidence rates for these disorders.9,10 We learned that incidence rates for diagnosable DSM disorders were maybe not as high as we would expect or matched our clinical experience, and there was a large variation in rates. This led to recognition of the need for different measures that were more sensitive to issues of burn survivors. We began to see the development of burn-specific measures, such as the Satisfaction With Appearance Scale, the Burn-Specific Health Scale, and the Burn-Specific Pain and Anxiety Scale.11,–13 As these measures were introduced into larger databases, we found some surprising results, specifically, large burns do not always lead to bad psychological outcomes, and conversely some with small burns struggled with emotional recovery." @default.
