- W2606151567 abstract "A45 The tumorgenesis is thought to be a multi-step process and involves genetic and epigenetic changes. In recent years, the increasing evidences demonstrate that inactivation of tumour suppressor genes (TSG) play a crucial role in the pathogenesis and development of cancer. Epigenetics refers to the study of heritable changes in gene expression that occur without a change in DNA sequence. In cancer, silencing of TSG by methylation in CpG islands of gene promoter is a common, early and tumor type-specific event. Thus, we study the role of epigenetic changes in initiation of cancer and its application. Aiming at developing early diagnostic and prognostic markers during treatment, we created a method “multiplex methylation specific-PCR (MMSP)” which reflects gene expression via the methylation status of tumor-specific patterns of TSGs on the samples from the direct body fluid close to the primary tumor, e.g. nasopharyngeal swab and mouth washing from nasopanryngeal carcinoma (NPC) and urine from bladder or prostate cancer patients. The expression level and pattern from different panels of marker could be semi-quantitatively evaluated via one PCR reaction. We take NPC as model for testing the methylation pattern of 6 genes including quality control simultaneously by MMSP from biopsies and swab/mouth washing of NPC patients, and evaluated the validity of detecting methylation from swab as an early diagnosis tool. The result from 43 matched Chinese NPC with biopsy and swab, 25 sporadic NPC biopsies and 24 normal control samples showed that detection rate of cancer is 85 % from swab samples, comparing to NPC biopsies which is regarded as 100%. The sensitivity of this assay is equivalent to 2 cells from 105 cells and the false positive and negative are less than 5%. A second panel of marker was confirmed in 24 matched prostate tissue samples, which histopathologically diagnosed as prostate tumor and benign. Consequently, the assay is being applied to urine samples of prostate/bladder patients for early diagnosis. Thus, MMSP is not only having the value of diagnosis and monitoring the treatment of cancer, but also generating new criteria for tumor classification from molecular base, opening the way to a host of innovative diagnostic, preventive and therapeutic strategies." @default.
