- W26062440 abstract "1814 Objectives The aim of this study was to investigate the clinical significance of incidental focal prostate uptake on FDG PET/CT in male subjects. Differences between malignant and benign uptake were also analyzed. Methods A total of 13,193 whole-body FDG PET/CT scans (1,811 for cancer screening, 11,382 for evaluating suspected or alleged cancer not in prostate) were retrospectively reviewed to find presence, location, multiplicity and SUVmax of focal prostate uptake, and combined calcification. Final diagnosis determined by serum PSA level and biopsy was compared with PET findings. Results Incidental focal prostate uptake was found in 140 of 13,193 scans (1.05%). Among them, 71 subjects with diagnostic confirmation were included for the further analysis. Prostate cancer was diagnosed in 9 of 71 subjects (Group I, 12.7%). The remaining 62 subjects (Group II) had no malignancy in prostate based on normal serum PSA level (n=56), or elevated serum PSA level with negative biopsy results (n=6). While all malignant lesions were peripherally located at prostate glands, 56.5% (35/62) of benign uptake was centrally located (p=0.005). Positive predictive value of peripheral focal uptake for malignancy was 25%. SUVmax, multiplicity and combined calcification were not significantly different between two groups. Conclusions Although incidental focal FDG uptake in prostate is not common, cancer incidence of focal uptake is not low. Therefore, it deserves further evaluation. The location of focal prostate uptake may be helpful to select high risk subjects for prostate cancer." @default.
