Il s’agissait d’une étude rétrospective d’une série de patientes, toutes symptomatiques en préopératoire (saignements anormaux et/ou douleurs pelviennes et/ou infertilité) avec un diagnostic d’isthmocèle large, opérées dans notre centre par cœlioscopie. L’intervention consistait en une exérèse de l’isthmocèle suivie d’une fermeture en 2 plans de la cicatrice. Le suivi des patientes a été effectué lors de la consultation postopératoire, à l’aide du dossier médical informatisé et de questionnaires et portait sur l’évolution des symptômes et de la fertilité. Au total, 9 patientes ont été incluses, souffrant toutes d’isthmocèles larges diagnostiquées par échographie endovaginale associée à l’hystéroscopie, hystérosalpingographie ou une IRM pelvienne. On retrouve une disparition de la symptomatologie chez 78 % des patientes. Il a été obtenu 4 grossesses spontanées chez 4 patientes, dont 3 présentaient une infertilité. Le suivi médian a été de sur 28 mois. La découverte d’une isthmocèle large chez une patiente avec une symptomatologie invalidante et un désir de grossesse peut faire envisager la prise en charge chirurgicale qui est une technique efficace et à faible risque opératoire. To study the effects of laparoscopic repair of isthmoceles acquired after a cesarean section on symptoms and fertility. This retrospective case series study included symptomatic women (abnormal uterine bleeding and/or pelvic pain and/or infertility) suffering from a large isthmocele and treated laparoscopically in our center. The surgical procedure consisted in resecting the pouch and suturing the scar in 2 layers. The patient follow-up has been realised through the postoperative control, the medical file and a survey, and concerned the evolution of the symptoms and fertility. Nine patients have been included, all suffering from large isthmoceles, diagnosed by transvaginal ultrasound associated with hysteroscopy, hysterosalpingography or MRI. Postoperatively, the symptoms disappeared in 78% of the patients. There have been 4 spontaneous pregnancies in 4 patients, 3 of them had been diagnosed with infertility. The median patient medical follow-up lasted 28 months. The diagnosis of a large isthmocele in patients suffering from invalidant symptoms and infertility should lead to consider a surgical treatment which is an efficient and surgically safe procedure." @default.
