- W2610202223 abstract "The aim of the study is to assess relationship between uteroplacental Doppler sonography findings and cerebral perfusion by diffusion-weighted magnetic resonance imaging (DWI) in fetuses with early- onset intrauterine growth restriction (IUGR). The prospective study included 54 pregnant women (gestational age: 28–34 weeks) who presented with fetal IUGR and 15 healthy pregnant women with normally developing fetus. Fetuses with IUGR were classified into 4 groups based on Doppler sonography findings: group 1 (n = 12), fetuses with umbilical artery (UA) pulsatility index (PI) > 95p; group 2 (n = 11), fetuses with UA PI > 95 p and middle cerebral artery (MCA) PI < 5p; group 3 (n = 21), fetuses with absent end-diastolic (EDF) in UA; group 4 (n = 10), fetuses with reverse EDF in UA. After Doppler evaluation, DWI was performed in all patients within hours. The groups were compared with respect to ADC levels. In cases with fetal IUGR, significant decreases were detected in ADC values of periatrial white matter (PAWM) (p = 0.006), thalamus (p = 0.002), basal ganglia (p = 0.007) and pons (p = 0.033) compared to controls. In Doppler subgroup analysis, the decrease in ADC values of frontal white matter (FWM) in the group 3 and 4 were found to be significant compared to remaining groups. ADC value of PAWM was decreased in the group 4 when compared to controls (1602.3 ± 68.8 vs 1794.6 ± 155.4, p = 0.007). Thalamic ADC values were lower in the group 2 and 3 when compared to controls (p = 0.017). No significant differences in ADC values were found between pons, basal ganglia and cerebellum among Doppler groups. In fetuses with IUGR, ADC values as measured by DWI are decreased. In some brain areas (FWM, PWM, thalamus), ADC values decrease significantly with worsening Doppler findings compared to those with milder Doppler abnormalities, and controls. Further prospective studies with larger sample size are needed to introduce cerebral ADC values in the management of IUGR." @default.
